Care Coordinator
Four Oaks Healthcare is a nurse-led domiciliary care and, due to expansion, we are looking for an enthusiastic, passionate individual to join our growing team as a care coordinator.
As the care coordinator, you will be responsible for managing a team of care assistants as well as maintaining a good communication line with all customers and other parties to ensure we deliver a high standard of care.
The ideal candidate will:
* Have a passion for delivering good quality, person-centred care
* Ideally have at least 2 years of experience within the domiciliary care sector
* Be flexible and reliable
* Be computer literate
* Be a good team player
* Be motivated and able to work on their own initiative
* Have exceptional people management skills
* Must be a driver with access to own vehicle
Duties Involved for the Care Coordinator:
* Preparing & scheduling rotas
* Writing and reviewing care plans
* Conducting risk assessments
* Interviewing potential candidates
* Conducting spot checks
* Being on call
