Support Worker Preston £13.65 per hour Full time Role Overview: Brook Street Social Care is seeking compassionate and motivated Support Workers to join our Day Services team in Preston. This role involves providing high-quality care and support to individuals with learning disabilities, helping them to live fulfilling lives with greater independence, choice, and control. You will support individuals in a person-centred way, encouraging participation in meaningful activities while promoting confidence, wellbeing, and social inclusion. Reporting and Key Relationships: Reports to: Assistant Managers, Manager, Disability Services Manager Works alongside: Colleagues, managers, and individuals accessing the day service Key Responsibilities: Support individuals to build confidence, self-esteem, and communication skills, enabling them to form relationships and engage in community life Create a welcoming, inclusive, and respectful environment that celebrates individuality Encourage and involve individuals in making decisions about their lives, supporting their overall health and wellbeing Develop and maintain positive working relationships with families, carers, and professionals Assist individuals in accessing activities in line with their person-centred plans Plan and deliver structured sessions and activities tailored to individual needs Maintain clear and accessible records of participation, progress, and development Attend review and planning meetings when required Support individuals to take part in age-appropriate activities within the local community Provide personal care and assist with ongoing health monitoring where necessary Offer physical support, including assisting those who use wheelchairs when out in the community Complete any required training to meet specific health or support needs Work collaboratively with families, carers, volunteers, and external professionals to enhance service delivery Act as an advocate to ensure individuals' voices are heard and respected Promote and uphold best practice in safeguarding, health and safety, and mental capacity Engage in regular supervision and contribute to team meetings Work in line with all organisational policies and procedures
